Privacy Notice

Who we are

Hale Medical Centre is a family medicine clinic at 6-875 Main Street E, Milton, Ontario, opening a second clinic at 80 Holland Street East, Bradford, Ontario. This notice covers information collected through halemedicalcentre.ca. Information you share during your care at the clinic is protected under Ontario's Personal Health Information Protection Act (PHIPA).

What we collect on this website

• Bradford waitlist: your name, email address and phone number, when you choose to join the waitlist.
• Online booking: appointment requests are handled by Ocean (CognisantMD), our secure booking provider, on its own website. Their privacy practices apply to information you enter there.
• Website analytics: we use Google Analytics to understand how visitors use our website, such as which pages are viewed. This uses cookies and does not tell us who you are.
• Maps: our pages show Google Maps, which is provided by Google and subject to Google's privacy policy.

Please do not include any health information in the waitlist form. It only asks for contact details.

How we use your waitlist details

We use your name, email address and phone number only to contact you about the opening of our Bradford clinic. We do not sell your information or use it for unrelated marketing.

When you join, we send you a confirmation email.

How your details are sent and stored

The waitlist form is processed by FormSubmit, a third-party form service. Your details are sent over an encrypted connection and delivered by email to the clinic's inbox, where access is limited to clinic staff. Because FormSubmit is located outside Canada, your details may be processed in another country.

How long we keep it

We keep waitlist details only as long as needed to contact you about the Bradford clinic opening, and then delete them.

Your choices

You can ask us at any time to see, correct or remove your waitlist details, or to take you off the waitlist. Just contact us using the details below.
